1.) Complete the Fax/Mail Order Form. Call if you have any questions.

2.) Print out the Fax/Mail Order Form.

3.) Sign the Fax/Mail Order Form - The signer must have the right to reproduce the art described herein.

4.) Send to Paradise-Giclee the following items.

• a. Completed Fax/ Mail Order Forml
  b. Original art or other media (If sending Computer Files, Slides or Transparencies view instructions.)
• c. When sending Original Art, please make sure it is packaged properly and insured.
• d. Enclose Deposit/Payment Check or Credit Card Information if not paying online.

5.) We will print proofs and ship them along with your original artwork within 5 business days for your final approving.

6.) After looking over the proofs and you are completely satisfied, send back the proofs with the approved print signed by you and marked "Approved". After your approval, and receipt of your final payment, we will proceed with final printing within 5 business days and ship directly to you.

7.) Initial deposit includes return UPS shipping of 1st proofs (insurance is extra)\

8.) Initial deposit does not include final order shipping charge of ($12.00 first piece & $2.00 ea. additional piece) plus insurance.
